Note from Ran Kailie: The email I received is below, however this survey is not offically sponsered or endorsed by House Eclipse. If anyone with the house is interested in a printed copy, I will make a request for some for you, please let me know. This seems to be well meant but again as it says do not provide ANY personally identifiable information if you decide to do this. The printed and mailed form is perhaps the most anoymous method since the online version could feasibly log IP address.

SangXUnion: Comment regarding good survey, completed, and asked about what others knew of AVA. Post since deleted since the forum changed formats on House Eclipse.
